Meals Direct helps eligible Medicaid Waiver / Community HealthChoices participants receive home-delivered meals at no cost through covered benefits. If you are not sure whether you have a waiver, this form can help our team review your information and explain possible next steps.
Free meal delivery is available only for people who qualify through a Medicaid Waiver-covered program. Approval is not guaranteed and depends on Medicaid / waiver eligibility requirements. If you want to order meals and pay out of pocket, please use our private-pay site: app.mealsdirectllc.net.

(267) 660-4020If you are NOT already waiver-approved, obtaining the waiver can take up to three (3) months or longer. During this process, various forms have to be completed by the applicant and their physician. The applicant will be interviewed three (3) times to verify eligibility, and five (5) years’ worth of bank statements must be submitted to verify income requirements. If these statements aren’t available, send as much of the required documentation needed and for anything missing, send a letter explaining why you do not have certian documentation. Meals Direct staff will help you walk through the process FREE of charge with written consent.
