If you are NOT already waiver-approved, obtaining the waiver can take up to three (3) months or longer. During this process, various forms have to be completed by the applicant and their physician. The applicant will be interviewed three (3) times to verify eligibility, and five (5) years’ worth of bank statements must be submitted to verify income requirements. If these statements aren’t available, send as much of the required documentation needed and for anything missing, send a letter explaining why you do not have certian documentation.
